Textbook Review Request Form

Midwest Theological Forum will provide complimentary review copies of its Didache Series textbooks to interested high schools upon request. Complimentary copies will only be sent if the school intends to review the textbooks for possible adoption in their courses.

Please fill out the fields below and select the titles necessary for the schools review. Upon verifying the information, the review copies will be sent to the school’s address. Please note that review copies will only be sent to the school directly – no home addresses.
